Terms of use

FRx Insurance is an independent reference site. It is provided for general informational and educational purposes for pharmacy professionals and related readers. It is not medical advice, legal advice, regulatory advice, billing advice, financial advice, or a substitute for current primary sources.

No affiliation

The site is not operated by, endorsed by, or affiliated with any insurer, adjudicator, pharmacy benefit manager, manufacturer, public program, government body, pharmacy chain, professional association, or regulatory college referenced in the content.

No warranty

The site is provided as is and as available. Content may be incomplete, outdated, or incorrect. Carrier rules, public program requirements, forms, PINs, URLs, and adjudication behaviour can change. Users are responsible for verifying information against current primary sources before relying on it.

Professional responsibility

Pharmacists and other healthcare professionals remain responsible for their own professional judgment, documentation, patient counselling, regulatory duties, employer policies, and current payer agreements. The site does not authorize any claim submission, intervention code, clinical service, or patient-care decision.

Limitation

To the maximum extent permitted by law, the site operator disclaims liability for losses or damages arising from use of the site, including claim rejection, audit recovery, clawback, patient harm, professional discipline, business interruption, or lost revenue.

Primary sources control

If FRx conflicts with a current Ministry publication, carrier manual, adjudicator response, regulatory requirement, employer policy, or prescriber instruction, the current authoritative source controls. The site should be corrected rather than used to argue against a payer or public program.

No emergency use

The site is not designed for emergencies or urgent clinical decisions. It does not provide patient-specific triage, diagnosis, therapy selection, or legal advice. Where a patient-care concern exists, the pharmacist’s professional obligations and local escalation pathways apply.

Permitted reference use

Users may use the site to locate topics, prepare questions for a payer, identify likely documentation requirements, and find source links. Users may not represent FRx as a payer authorization, official public program statement, or carrier endorsement.

Changes to the site

FRx may update, remove, reorganize, or rewrite content when source material changes or when a page is found to be unclear. A guide may describe a workflow at a point in time while a payer later changes its form, portal, or adjudication response. Users should not rely on a cached or printed copy without checking the current page and source.

External links

External links are provided to help readers verify source material. FRx is not responsible for the content, availability, accuracy, privacy practices, or policy changes of external sites. A broken link should be reported as a correction, but a broken link does not make the older statement current.

Advertising

Advertising, if present, does not affect the editorial status of a guide. An advertisement or sponsored placement does not indicate endorsement by FRx and does not change the requirement to verify pharmacy billing information against current primary sources.
